JOB DESCRIPTION:PHYSICAL THERAPIST Title: Physical TherapistLocation: Story Place Preschool, Inc.Person Responsible To: Preschool Leader and Story Place Preschool, Inc.Number of Staff Supervised: 0Education and/or Experience Prerequisites:Bachelor of Science in Physical Therapy, New York State license/registration and Pediatric experience preferred. Basic Function:Evaluate the need for and provide services in physical therapy for identified children with motor goals written on their IFSP/IEP. Responsibilities:Perform diagnostic testing to evaluate the need for programs of physical therapy.Develop and update physical therapy goals for each child. Participate in team planning.Provide the PT input in IFSP/IEP planning.Receive input from other team members.Educate staff in aspects of PT through in-service.Maintain written and oral communication with parents/caregivers.Collaborate with therapists, family service providers, and health services specialists.Supervise PT staff and act as the clinical coordinator of PT interns.Consult and collaborate with other professional staff in evaluating and planning care for children.Attend designated meetings associated with this position.Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establish personal networks; participates in relevant in-services and conferences.Adhere to the policies and procedures established by Story Place Preschool, Inc. in the Employee HandbookProvide assistance to the Preschool Leader as requested.
